Switch to npm trusted publishing (OIDC) - no tokens needed

- Add id-token: write permission for OIDC
- Upgrade to Node.js 24 (npm 11.5.1+)
- Use --provenance --access public for npm publish
- Remove NPM_TOKEN dependency
